Holly- Lopez-Velasco joined MetroPlus in 2003. She developed the Diabetes Care Program and
is highly effective in her role in working with both members and providers in providing diabetes
case management.
Starting with the initial assessment, she continually advises members to help them manage their
condition successfully. She schedules appointments and expedites orders for new prescriptions
or DME, as well as keeping each patient’s doctor fully informed. She effectively coordinates
care for patients. She can easily develop working relationships with members and providers and
involve other MetroPlus case managers when appropriate.
Ms. Lopez-Velasco received her R.N. from Beth Israel School of Nursing. She has worked as a
Staff Nurse, Preceptor, and Assistant Nursing Care Coordinator at various hospitals throughout
New York City. Her list of accomplishments include working as a Clinical Manager for
Retrospective Review for Capital Equipment and Transplants at UMD/CMD and working as
an Assistant Director for Health Insurance Plan of NY (HIP) in their Grievance and Appeal
Division. She also worked for United Healthcare in positions related to Care Coordination,
Patient Advocacy, and Government Programs. She also worked at Guild Net, New York, as a
Case Manager in the Long Term Medicaid Managed Care Program.
